IMM 5476: Use of a Representative

IMM 5476 authorizes someone, a lawyer, consultant, or other representative, to act, communicate, and receive information on an applicant's behalf with IRCC.

What to know before you fill it out

It's only needed when someone other than the applicant will communicate with IRCC about the file. Both the applicant's and the representative's sections need to be completed, and it's worth keeping a signed copy for your own records.
